Simple but Not Easy

This week, I authored a guest post in Meidas+ on the politician at the Food and Drug Administration (FDA) who overruled career scientists to block Moderna’s flu vaccine application from review. You can read the full piece below on how FDA Commissioner Marty Makary must be fired.


“In the long contest between biology and bullshit, biology is undefeated,” Auchincloss [told Bloomberg]. “The [FDA] commissioner is trying to wage that battle, and it’s going to be patients who lose.”

The FDA is staffed by scientists who review evidence to determine the safety and efficacy of medicines. It’s a bureaucracy wrestling with biology, so of course it’s messy. But in general, they have done well. Away from the political spotlight, the FDA sifted through randomized controlled trials and debated surrogate endpoints with investigators. It became the world’s gold standard biomedical regulator.

Until 2025. Then, the bullshit started. A crew of anti-vax zealots, bio-hackers, and wellness grifters descended on the Department of Health & Human Services. Now, measles is breaking out; AI slop is replacing scientific review; the scientist who ensures your eye drops don’t blind you was fired; and the wealthy receive tax breaks for buying bidets.

The GOP is too afraid of RFK. Jr to hold a hearing. Big Pharma is rushing to the White House to cut pricing deals, averting its eyes from the FDA en route. While industry caves and Republicans shrug, the FDA has hemorrhaged competence and credibility. A former commissioner — who served in a Republican Administration — told me the FDA had lost 20 years in the last one.


Last week, a political hack, Vinay Prasad, overruled the FDA’s top vaccine scientist to reject Moderna’s mRNA vaccine. Safety and efficacy lost out to fear and favor. The FDA just reversed itself, but the damage to public health and biomedical innovation from raw politics is profound. mRNA is the ‘software of life’, translating genetic code into the proteins that are the cell’s machinery. Engineering mRNA can defend against infectious disease, regenerate tissue, and even inoculate you against cancer. But this Administration refuses to research and regulate mRNA’s potential.

I represent a lot of scientists. I serve on the committee of jurisdiction for FDA issues. I’ve been on the warpath for the last year, but only in the last month has my pushback gotten traction. Democrats must sink our cleats into the foothold we’ve gotten and push off on a three-part FDA agenda of accountability, integrity, and innovation.


Accountability, first. Commissioner Marty Makary needs to be fired. Vinay Prasad needs to be fired. And any FDA official complicit in RFK Jr.’s anti-vax crusade needs to be fired. That starts with congressional oversight hearings, now. I’ve been working for months to build up pressure for them. If Democrats win the midterms, those hearings can become proper investigations.

Democrats must also restore the integrity of the FDA. The Commissioner’s National Priority Voucher program, which expedites drug reviews at the commissioner’s say-so, rewards companies with ties to MAGA over scientific breakthroughs. Every voucher handed out under this illegal program must be re-examined. It is a political end-run around scientists.

We must install new leadership that respects the firewall between the FDA and the White House, that recruits and empowers top-notch scientists to regulate industry, and that focuses on how to examine evidence on the safety and efficacy of medicine – without being distracted by wellness hashtags. (This includes issues fashionable on our side of the aisle, too. The non-GMO movement, for example, is divorced from any evidence about the safety or efficacy of genetically modified organisms.)

An FDA restored to accountability and integrity is a boon to innovators and a tax on snake-oil salesmen. Just as chemistry became an engineerable discipline in the 19th century, and physics in the 20th, biology can become an engineerable discipline in the 21st century. From food to fuel, materials to medicine, biotechnology will do wonders. But only if regulators follow the science, not RFK Jr.

Thank you for contacting me to express your support for the Supplemental Nutrition Assistance Program (SNAP). I appreciate your input on this important issue and welcome the opportunity to respond to you directly.

 

I share your support for food assistance programs, particularly the Supplemental Nutrition Assistance Program (SNAP), and I oppose the Trump Administration’s attacks on the program. I joined my colleagues in sending a letter to USDA Secretary Brooke Rollins on October 24 urging the agency to use its contingency reserve, and its legal authority to supplement these funds, to fully finance November SNAP benefits while the government remained shut down. Unfortunately, multiple states, including Massachusetts, were forced to pause benefits on November 1 due to USDA’s refusal to distribute this funding and fulfill its legal requirement to do so.

 

Massachusetts has since resumed benefits following the reopening of the federal government on November 12 and was able to begin distributing partial benefits even sooner due to federal court orders directing the Trump Administration to fund November payments. The Healey-Driscoll Administration published a Frequently Asked Questions (FAQ) webpage, which includes important resources regarding who qualifies for the program and how to access support here: mass.gov/info-details/november-2025-snap-updates .

 

You may be pleased to know that I have taken steps to defend SNAP against the Trump Administration’s efforts to weaken it. I voted against the House Republican reconciliation bill, which President Trump signed into law on July 4, 2025. The bill will slash SNAP by expanding unnecessary paperwork requirements and requiring states to pay a portion of food benefit costs. In Massachusetts, more than one million people rely on SNAP, and roughly 175,000 could lose some or all of their benefits as a result of this legislation. The Greater Boston Food Bank estimates it would take $98 million annually for the state’s food banks and pantries to make up for the loss.

 

In addition to opposing these harmful cuts, I have taken further action to support and expand upon the essential work that these programs do to reduce hunger in the United States. I am a cosponsor of H.R. 2512, the Hot Foods Act. This bill would remove the prohibition on purchasing hot foods ready for immediate consumption from SNAP. Last Congress, I cosponsored the Improving Access to Nutrition Act, which would have lifted SNAP’s three-month limit for certain adults.

 

I have consistently opposed legislation that would cut these essential programs, including the House Republican Farm Bill that would have cut roughly $30 billion in benefits for all SNAP participants in future years. I appreciate your advocacy for food security, and I will continue working to ensure that SNAP benefits are protected.

Medicaid waivers: the bigger health care fight to come

Health care is the pivotal issue of the shutdown debate. In particular: enhanced premium tax credits for Affordable Care Act plans. The health insurance policy details are wearisomely complicated – and important.

But not as complicated and important as Medicaid waivers. These wonky waivers are the best chance Democrats have to govern well on health care during the Trump administration. They should be central to our health care agenda.

The waivers are formally called Section 1115 Demonstrations. They are state-by-state agreements with Washington that give state Medicaid agencies flexibility in how they deliver and pay for health care. More than 40 states are up for re-negotiation during the Trump administration.

Under the wrong terms of negotiation, there will be cuts and cruelty. Premiums for the middle class in group insurance markets will go up even more. Per-patient spending is already climbing by double-digit percentage rates. Health premiums are rising twice as fast as wages.

Under better terms, the negotiations could be a starting point to lower costs and reduce bureaucracy. Using Massachusetts as an example, I wrote recently for The Boston Globe about how state & federal Democrats can seize these waiver negotiations to deliver on better health care.

Here are excerpts:

President Trump’s One Big Beautiful Bill slashes at least $2 billion annually in federal funding for MassHealth [and] adds... eligibility paperwork that will kick hundreds of thousands of adults off coverage. This will raise emergency room wait times and insurance premiums by hundreds of dollars a month for all of us.

Rather than accept less money and more rules, MassHealth’s new [Section 1115] proposal should offer a radical deal:

1. MassHealth accepts an inflation-protected grant from Washington.

2. In return, MassHealth gets to make its own decisions about how to reduce costs and increase quality. And,

3. MassHealth gets bonus payments if its interventions reduce costs for Medicare, the federal health insurer for Americans over 65.

...

There’s risk, but the rewards are greater. MassHealth could deliver better care, particularly primary, behavioral, and elder services. And it could contain costs.

Freed from federal rules and bureaucracy, MassHealth could braid together benefits. Medicaid, welfare, food stamps, home-heating relief, and rental assistance all put federal dollars in reach of the same population. MassHealth could streamline eligibility and enrollment. It could laser focus on impact for children. It already insures 4 in 10 children in Massachusetts, so MassHealth is well-positioned to quarterback the full playbook of services that improve children’s health and wellness.

Fewer restrictions from Medicaid would also unlock pilot programs. MassHealth is the primary payer for mental health and addiction services in the state. Massachusetts is home to world-leading innovation in both...

MassHealth is already making strides in paying for value instead of volume. It is two years into a program in which it pays the state’s 17 accountable care organizations (ACOs) for patients’ total cost of care. ACOs are organizations led by doctors and other health care providers, and they are financially accountable for the quality and cost of the care. This experiment is meant to drive a shift away from fee-for-service medicine toward more investment in primary and preventive care, in particular... MassHealth could experiment and improve faster with more flexibility than the federal government allows now.

Some of those improvements will save Medicare money. Medicare Part A pays for hospital care for the over-65 population. Part B pays for outpatient treatment, like doctor visits or ambulance services. MassHealth pays for long-term services and supports (LTSS), either at home or in nursing facilities, for that same population. When MassHealth innovates with LTSS to reduce hospital visits, Medicare saves money. But MassHealth gets no share in those savings.

Massachusetts has always been at the cutting edge of health care. Democrats have always prioritized health care. Democrats in Massachusetts... can show the nation a better way on health care — just as we did with the Affordable Care Act — that delivers quality primary, behavioral, and elder care, at lower cost.

Thank you for contacting me to express your support for the Head Start and Early Head Start programs. I appreciate your input on this important issue and welcome the opportunity to respond to you directly.

Like you, I am a strong supporter of Head Start, which provides early learning and development, health, and family well-being services to promote the school readiness of children from low-income families. Head Start has served nearly 40 million children nationwide since its founding, and, in Massachusetts, Head Start programs served over 11,000 children and employed over 4,000 people in the 2023-2024 program year. As the father of three young children, I understand firsthand how essential early childhood education is to ensuring every child has a strong foundation for academic, social, and emotional growth throughout their life.

I am committed to providing access to child care for all Americans. That is why I joined my colleagues in sending letters to the House Appropriations Committee requesting robust funding for the Preschool Development Grant Birth through Five program (PDG) and Head Start in the Fiscal Year (FY) 2026 spending bills. On March 11, 2025, the House passed H.R. 1968, a continuing resolution (CR) to fund the government through September 30. The CR maintains funding for these programs at FY 2024 levels, which authorized and appropriated $315 million for PDG and $12.27 billion for Head Start.

Further, you may be interested to know that I strongly support direct-to-provider operations grants for Massachusetts. These grants, initially created with funding provided through the American Rescue Plan Act, provide monthly allocations of public funds to early childhood education providers based largely on the number of children that a provider is licensed to serve. These grants also provide additional funding to programs that serve low-income children and that employ more educators, incentivizing lower ratios of children to teachers and thus improving the quality of early learning programs. Direct-to-provider grants have worked as intended, allowing providers to stay open, increase teacher pay, and minimize tuition hikes.

Funding for high-quality child care and early education is an investment in American families and our economy. As your Representative, supporting families with young children is extremely important to me. Please be assured that I will keep your views in mind should legislation related to Head Start be considered in the U.S. House of Representatives.
